During morning observations, The client appeared to have difficulty with following directions during his morning stations. During midday observations, he interacted with his peers in a positive manner during class reading time. During afternoon observations, The client was able to remain focused while completing his math test. QMHP-C greeted the client upon arrival to the school setting. QMHP-C inquired about the client’s morning. During morning observations, QMHP-C redirected The client and reiterated the importance of following directions from his teacher. During midday observations, QMHP-C provided The client with positive reinforcement and encouraged him to continue to interact with his peers in a positive manner. QMHP-C provided The client with verbal praise and prompted him to remain focused while completing his test. The client responded well towards Therapeutic Day Treatment services throughout the day as evidenced by participation and positive feedback. During morning observations, The client was receptive to redirection and agreed to follow directions while doing station work. During midday observations, The client agreed to continue to interact with his peers in a positive manner. During afternoon observations, The client continued to remain focused and ignore distractions while testing. He agreed to continue to track his behaviors on his self-monitoring chart.
